Registration for all Fall sports will open on July 1st.

1. Current physical turned into the main office. We recommend that you receive a new physical each summer so it doesn't expire.
2. Update your emergency contact information through the registration process so your coach has the most up-to-date contact information.
3. Pay an Athletic Fee of $130: Can be paid at PSD Athletic Registration Or in the front office.
4. For each practice you will need: shorts, a t-shirt, a water bottle, running shoes, and other weather-appropriate attire/football equipment. Games schedules can be found at: Boltz Football Game Schedule Also follow the calendar on the Boltz home page for game updates.

Students who qualify for free/reduced lunch must have a new fee waiver on file each year in order to have fees waived. We can assist parents with this process during check-in. It is a two-step process with applying for free/reduced lunch first, then applying for additional fees to be waived (on the Parent View “fees” tab).

A signed Physical Form must be filled out by a physician before a student can play a sport. Below are some low-cost resources.

The Health and Wellness Center at Centennial High School offers sports physicals/well-child exams June - August. The Center can bill insurance and no child has ever been turned away for inability to pay. For more information visit their website or call their office at 970-488-4950.
